TTM Nursing Compliance
Please find below a list of the certified documentation we require prior to you working as a TTM Nursing locum.
Garda Vetting Application Form & Payment. Please return this immediately, don't wait until you have the rest of your paperwork, as they are currently taking 16 weeks to process. The application form MUST be printed double sided. We cannot accept the application on two separate pages, nor can we accept photocopies.
We require Police Clearance Certification for every country that you have lived in for 6 months or more since the age of 18, regardless of your employment status in that country.
TTM Nursing Application form needs to be completed in full
The following documents are required and must be returned with the completed registration form to the Compliance Department at TTM.
- Copy of current registration with An Bord Altranais
- Copy of Garda National Immigration Bureau Card ( where applicable)
- Copy of passport **
- CPR Certificate valid within the last two years**
- Patient Moving and Handling Certificate valid within the last two years**
- Fire Safety certificate ( optional)
- 2 passport photographs (Agency Nurses only)
- Immunisation certificates **
- Copy of Nursing Qualifications **
- Copy of Union Membership (Agency Nurses only)
- Completed Garda Vetting form
- Evidence of PPS Number - e.g. P45, P60, Notice of Tax Credits, government letterhead stating your PPS**
- €15 cheque /postal order for Garda Vetting process
- Infection Control certificate **
The documents indicated above with ** must be certified as true photocopies. To do this, bring the original and a photocopy to your local Police Station. Once you have all your documents post them to TTM Healthcare, Ballymaley Business Park, Ennis, Co. Clare.
